Accreditation Process
Pursuant to Section 54 (1) of the Health Professions Act No. 24 of 2009 of the Laws of Zambia, a health facility shall apply for accreditation of a health care service to the Council in the prescribed manner and form upon payment of the prescribed fee.
- Download application form for accreditation of health services
- Complete the application form
- Conduct a self-assessment for the service applied for
- Pay the prescribed license fees
- Submit application form with all supporting documents including a letter inviting the Council to inspect the facility clearly stating the proposed date of inspection
- The Registrar appoints an inspection team to inspect the health facility
- Inspection team submits reports to registrar
- Registrar recommends to the Licencing and Accreditation committee for:
a) Approval if the facility meets minimum requirements
b) Rejection if the facility does not meet the minimum requirements
Licencing and Accreditation committee
a) Recommends to the Council Board for approval
b) Rejects the application and the Council issues a rejection notice
Board approves the recommendation to grant:
a)Provisional Accreditation certificate
b) Full Accreditation certificate
